An innovative 'joined-up' approach to elderly health care was started in Hillingdon. The Hillingdon Health Care Partnership (HHCP) brings hospital services, GPs and community care together in Care Connection Teams, designed to help prevent emergency admissions to hospital among older residents.

Due to there being more than one provider for this service, CNWL headed up creating the new logo for the team.
