The challenge:
There is a risk of a member of the public falling if they are taking numerous medications. The term used to describe the concurrent use of multiple medications by a patient is Polypharmacy. 1000 Lives and Public Health Wales need to Promote the Medication Review procedure, to encourage patients to have their medications reviewed with either a GP or a Practice Pharmacist, to ensure their safety and reduce the risk of falls.

Solution:
The Steady On print and digital campaign main purpose is to encourage anybody who is on several medications to request a medication review and help to reduce the risks of falls in the UK. So the goal was to raise awareness about the subject matter and allow easy access to the information about polypharmacy by creating a simple and easy to use online platform with a straight forward booking system. Also, the webpage provides additional programs and articles to help the patient with keeping on track with their medicine and improve on their strength and overall well-being.
